So schedule your visit as soon as you can.

The end of summer means going back to school and the doctor. Getting an annual checkup is important for school-age kids and college students. These checkups help you stay healthy by completing important preventive care screenings. They can also connect you or your child to health care resources. What to talk about with your doctor: The doctor can answer questions about the following:

*The adolescent vaccine series includes the human papillomavirus (HPV), Tdap and meningococcal (MCV); all should be completed between ages 9 and 13.

Earn rewards You can earn a gift card to Target, Walmart, or Amazon for completing one of these health care exams in 2023: ● Well-care exam: Members 3 to 21 years of age can earn a $25 gift card. ● Cervical cancer screening (Pap test): Female members 21 to 64 years of age can earn a $25 gift card. ● Breast cancer screening (mammogram):

Female members 50 to 74 years of age can earn a $40 gift card. You can find the member reward forms on our website at goldcoasthealthplan.org under “For Members” > “Member Resources” > “Member Rewards.” Once you have completed the exam, fill out the form.
